I'm pants at it, but basics are as follows
Run in windowed mode (1280x720) (so it's in HD res)
host_framerate 100 (can go higher or lower than 100)
Startmovie moviename avi/tga/H264 (most choose H264)
Let source do its magic and save the video
Can upload like that, or open in editing software (I use sony vegas 10)
Render as HD video.
